abstract = {Introduction: Liver cirrhosis is an irreversible progressive, condition characterized by widespread hepatic fibrosis, hepatic architecture distortion and formation of regenerative nodules, which finally leads to liver failure. Globally, liver cirrhosis ranks as the 11th leading cause of death globally, contributing to 2.2% of all deaths1. One report from 2023, citing the Global Burden of Disease study, states that India recorded the highest number of deaths from cirrhosis and other chronic liver diseases in 2017, with approximately 0.22 million deaths2. The most common causes of cirrhosis worldwide are related to viral hepatitis, alcohol, and non-alcoholic fatty liver disease. Besides Hepatotropic viruses, which are the aetiological factor in most cases of acute hepatitis, drug-induced liver injury also increasingly accounts for a significant proportion of cases. Liver cirrhosis is characterized by hepatic fibrosis, portal hypertension and functional decline. Despite advancements in conventional medicine, management remains largely supportive. Ayurveda offers a unique, individualized, holistic approach that may complement modern treatment modalities. Modern medicine primarily offers symptomatic and supportive treatment for cirrhosis, such as management of complications (ascites, hepatic encephalopathy, variceal bleeding), lifestyle modifications, antiviral therapy, and in end-stage cases, liver transplantation. However, limitations exist due to the lack of curative pharmacological agents and the inaccessibility of transplantation for most patients due to cost, availability, or contraindications. Ayurveda being the traditional system of medicine practiced in India, conceptualizes liver disorders under the broad category of "Yakrit Roga," with conditions like Kumbha Kamala and Udara Roga sharing similarities with cirrhosis in terms of symptomatology and pathogenesis. Ayurvedic management thus focuses not only on holistic healing through Shodhana , Shamana and Pathya Apathya modifications, aiming to restore balance among the Doshas (Vata, Pitta, Kapha) and enhance hepatic function.
Aim: To Present a case of liver cirrhosis (Decompensated) managed successfully through an integrative approach combining Ayurvedic and Modern medicine interventions.
Case Summary: A 40-year-old female diagnosed with autoimmune hepatitis (decompensated liver cirrhosis) was treated using a multidisciplinary approach. Ayurvedic interventions included classical formulations (Bhumyamalaki, Punarnava, Katuki, Vardhamana Pippali, etc.), Virechana therapy, and strict dietetic and lifestyle protocols. Modern treatment included diuretics, hepatoprotective agents, and monitoring of liver function parameters. Over 6 months, the patient showed substantial clinical and biochemical improvement.
Conclusion: The integrative approach provided symptomatic relief, improved liver function, changes in fibroscan dimensions and enhanced quality of life. Such integrative care protocols may serve as supportive strategies in cirrhosis management and warrant further clinical investigation.},
