abstract = {Background: Endometriosis is a chronic estrogen-dependent gynecological disorder characterized by ectopic endometrial tissue deposition resulting in pelvic pain, dysmenorrhea, dyspareunia, and infertility. Conventional treatment strategies emphasize hormonal suppression and surgical excision; however, recurrence rates are high and fertility outcomes remain suboptimal. Ayurveda offers a comprehensive approach to reproductive pathology through modulation of dosha, dhatu, and srotas, and provides non-invasive fertility-restoring options for women attempting natural conception. Case Description: A 29-year-old female with a 3-year history of primary infertility and severe dysmenorrhea was diagnosed with left ovarian endometriotic cyst (4.2 cm) and mild posterior cul-de-sac adhesions. Hormonal profile was unremarkable except for mildly elevated CA-125. She discontinued prior hormonal suppression therapy due to side effects and desire to conceive. Ayurvedic assessment revealed Vata–Kapha dominance with Ama, Srotorodha, and Artavavaha Srotas involvement. Intervention: Treatment was delivered in three sequential phases: (1) Deepana–Pachana for Ama reduction, (2) Shodhana and Srotoshodhana via Kshara Basti, Uttarabasti, and Guggulu-based Granthi-hara formulations, and (3) Rasayana and Garbhasambhava-samskara for ovarian–endometrial optimization. Dietary regulation and timed coitus counseling supplemented the protocol. Outcomes: Significant reduction in pain and menstrual abnormalities was observed by the 3rd month, with improved follicular maturation and ovulation confirmed by ultrasonography in the 5th month. Natural conception was achieved in the 6th month, followed by uneventful antenatal course and full-term vaginal delivery of a healthy newborn.},
