A SLIM PRICE TO PAY? A RARE ESOPHAGEAL COMPLICATION OF TIRZEPATIDE

  • A 42-year-old male with no significant comorbidities presented with hematemesis and a transient loss of consciousness. He had recently initiated pharmacologic therapy for weight loss and was on the fourth dose at the time of presentation. Upper gastrointestinal endoscopy revealed a Mallory-Weiss tear as the source of bleeding. While such tears are classically associated with forceful vomiting or retching, the temporal relationship between symptom onset and initiation of weight-loss therapy raises consideration of contributing factors. This case underscores the need for clinical awareness of gastrointestinal mucosal injury in patients receiving newer anti-obesity medications and highlights the importance of early recognition and supportive management in preventing complications.
